Hey @DanielRausch – There is cool tool that I discovered that I thought I would share with you as well that will be helpful with debugging this. It is called “Preference List Debugging” and can be found in the Session Information Report in the Additional Support Reports section. Basically, it shows all the preference lists in use for the current user and patient encounter. For more information on exactly what the tool does, here is the Galaxy guide that describes it in more detail: Troubleshoot Preference List Build with a Summary Report.
Based off of that, as an example, I went into the test patient Pediatrics C. Clindoc (MRN 204860) to see what was compiling for them, and I am not finding the preference list that you specified in the first post, 3040000019. I cannot tell whether it is that one of the preference lists at a more specific level, like location or department, are overriding this preference list, or that the preference lists you tried out are not attached to the facility you want it available for. See the screenshot below for the Preference List Debugging for that test patient I specified.

Overall, I think that the first thing we need to determine is what end users need the order available. Depending on that, we can identify an EAF or DEP record to dive into (and/or use the Preference List Debugging tool to help out in determining how the preference lists are compiling for a user that needs PRO501), and find what preference lists are available for them. We can then add PRO501 to that specific preference list and test from there to ensure it is not overridden at a more specific level.
